ABSTRACT

Distal anterior cerebral artery [ACA] aneurysms are rare and compose less than 5% of all intracranial aneurysms. Most of the aneurysms in this location are congenital, saccular ones, but it is also a well-known location for traumatic aneurysm. The mechanism of aneurysm development, clinical scenario and/or surgical difficulty are different between these two groups. Between March 2005 to February 2006, 6 cases of distal ACA aneurysms were treated at the Department of Neurosurgery, Fujita Health University, Aichi Prefecture, Japan. The clinical presentations, neurological findings, and treatment were analyzed. Of 6 patients, 2 were male and 4 were female, aged from 66-79 years. Of 6 cases, one was traumatic aneurysm, which occurred 2 months after a fall injury. Of the 5 congenital aneurysms, one was unruptured, the other 4 were all ruptured. One patient had multiple aneurysms. We believe that there are differences between the congenital and traumatic aneurysms in respect to the mechanism of development, aneurysm location, and treatment difficulties

ABSTRACT

Intracranial aneurysms in neonatal period and early infancy may be resorbed spontaneously or due to removal of predisposing factors. Herein, we presented a rare case of vanishing of a large thrombosed intracranial aneurysm in a 25-day-old girl presented with head enlargement and vomiting. Magnetic resonance imaging [MRI] revealed a severe hydrocephalus accompanied by a 1.5-cm aneurysm at the bifurcation of the right middle cerebral artery [MCA]. Doppler ultrasonography confirmed a flow with a magnitude almost similar to that of the right MCA flow, inside the mass located adjacent to it. The hydrocephalus was managed by ventriculo-peritoneal [VP] shunt. The patient underwent a conservative observation for several months. Subsequently, the aneurysm was vanished in control images

ABSTRACT

Vein of Galen aneurysm is a rare pathology, representing less than 1 percent of intracranial vascular malformations. We report on a 65 years-old man who experienced a generalized tonic-clonic seizure. Brain imaging showed a large calcified expanding mass in the pineal region, confirming the diagnosis of a vein of Galen aneurysm. Because of the spontaneous thrombosis of the malformation, there was no need for microsurgical or endovascular treatment and he is been regularly followed since that.


Aneurisma da veia de Galeno é patologia rara, representando menos de 1 por cento das malformações vasculares intracranianas. Apresentamos o caso de um homem de 65 anos que teve episódio de crise convulsiva tônico-clônica generalizada. Exames de imagem evidenciaram grande processo expansivo calcificado na região pineal, confirmando o diagnóstico de aneurisma trombosado de veia de Galeno. Devido à trombose espontânea da malformação, foi excluída a possibilidade de tratamento endovascular bem como microcirúrgico, mantendo-se o acompanhamento clínico.

ABSTRACT

In neonates, aneurysm of the vein of Galen often masquerades as cyanotic congenital heart disease. We report 4 cases of neonates presenting with malformation of the vein of Galen at our insititution. An increased awareness of this entity seems warranted.

O autor apresenta uma revisão atual sobre a hemorragia subaracnóide (HSA), enfatizando os mecanismos etiopatogênicos e fisiopatológicos, as manifestações clínicas, diagnóstico, as normas gerais de tratamento clínico, cirúrgico e através da neurorradiologia intervencionista, as complicações e o prognóstico dos pacientes, particularmente da HSA por rotura aneurismal.

Se trata el caso de un Aneurisma Gigante de la Vena de Galeno, en una preescolar de 2 años de edad, tratada por el Servicio de Neurocirugía del Hospital Miguel Pérez Carreño de Caracas. Dada la baja incidencia de esta patología, reviste un especial interés el hecho de diagnosticarla precozmente, por además presenta una alta mortalidad. Se realiza una revisión de la literatura médica y una discusión sobre los hallazgos imagenológicos, tratamiento y pronóstico
